Chronic open wounds are always contaminated with bacteria, and as they mature, become progressively more fibrous and avascular. Systemic responses to chronic large open wounds include blood volume reduction, increased peripheral vascular resistance and a hypercoagulable state - bad for healing and particularly troublesome for free flaps.
